Blake Garrett, the former child actor recognized for his role in the film "How to Eat Fried Worms," died due to an acute fentanyl overdose.

The Oklahoma Office of the Chief Medical Examiner's summary report confirmed the cause of death, ruling it an accident.

This development provides clarity regarding the passing of Garrett, who was 33 years old.

According to the report obtained by TMZ, Garrett died at a sober living house located in Tulsa.

His mother, Carol Garrett, had previously informed TMZ in February that her son had recently been diagnosed with shingles.

At the time, she speculated that he might have accidentally overdosed while attempting to manage pain, following three years of sobriety.

Born in Austin, Texas, Blake Garrett established an acting career early in his life.

He secured lead roles in local productions such as "Aladdin and His Magical Lamp" and "Peanuts: A Charlie Brown Tribute." At the age of 10, he joined the "Barney's Colorful World International Tour."

His most notable role came in 2006 when he portrayed the character Plug in "How to Eat Fried Worms." His performance in the film contributed to the cast receiving a Young Artist Award for Best Young Ensemble Cast.
